- When someone that is on your form is no longer a signer.
- When your church opens a new account.
- When contact information for signers or church has changed.

Why do you need an addendum form?
- When your church has changed their name or is in the process of a name change.
What else do you need to know about the form?
- Please make sure you fill out a separate form for each account your church holds with the Foundation.
- Also be sure you have the investment agreement and signature card on hand for each of the accounts the church holds with the Foundation. This will make filling out the addendum form easier.

UNDERGRADUATE SCHOLARSHIP

We offer a number of scholarships to students who are attending college for their undergraduate degrees. To qualify you must:
- Be a full-time student
- Have strong academic performance
- Be active in a local Methodist Church of Wesleyan origin
- Preference is given to those attending a Methodist college. Some scholarships are reserved for the children of Methodist clergy.
The application deadline is March 1. 

grants for church outreach programs

We welcome requests for the church to be in mission to its local community. Feeding programs, health and wellness programs, services for children and adults at risk and educational opportunities have been funded in the past. We do not fund requests for capital improvements that are not mission-related, debt retirement, or operating funds.
